The government assumes no financial responsibility for any costs associated with any response to this notice as incurred by prospective contractors. ************************************************************************** Project Title: Catawba Podiatry Service Tracking Number: 285-RFQ-24-0017 The Indian Health Service (IHS), Catawba Service Unit, in Rock Hills, South Carolina 29730 is seeking capable sources for Podiatry Services. The anticipated Period of Performance date is: Date of Award. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 621391-Offices of Podiatrists. The purpose of this sourcse sought is to find a Native Owned reliable source for Diabetic Shoe and Diabetic Shoe Insert services for Diabetic Patients at the Catawba Service Unit. That service shall be comprehensive and shall include the activities that follow with an appropriate referral enter into RCIS referral system by appropriate providers at Catawba Service Unit: Patient will be fitted for a pair of Diabetic Shoe or Diabetic Inserts once a year. A Diabetic patient may get a pair of each yearly dependent on medical necessity with approval from their provider and nutritionist. The contractor must be able to provide follow up for adjustments and concerns from the patient. The governmentwill evaluate market information to ascertain potential market capacity to provide supplies with those described in this notice. In accordance with the Buy Indian Act, 25 U.S.C. 47, the Indian Health Service shall give preference at all times, as far as practicable, to Indian economic enterprises. The contractor will evaluate and design a treatment plan for each individual who attends the clinic. The plan may include; extra depth or custom shoes and /or molded inserts. One pair of shoes and up to three inserts will be provided annually, unless otherwise indicated. Measurements will be taken at the time of the evaluation and prescribed device(s) ordered. The device(s) will be available for pick up the following month. The contractor, along with the Service Unit will plan and provide group and /or individual education as it relates to Diabetic foot care. The deliverables are custom prescribed orthotic footwear. One pair of shoes and up to three pair of inserts will be provided to each patient annually. To qualify for footwear coverage, Medicare beneficiaries must have diabetes plus one of the following conditions: Neuropathy with evidence of callus, previous or current ulcer, previous or current pre-ulcerative callus, previous, amputation, foot deformities, or poor circulation. The necessary information will be submitted on the form completed by the service unit M.D. The proposed number of patients to be serviced is approximately 60. The contractor shall be licensed in the state of South Carolina.
