## Description

This solicitation has been amended to bring awareness to pg 2 of Attachment 1-Contract Pricing and Subcontract Certification in the solicitation, which lists General Surgeon 3 times under "Other Physicians". This is incorrect. Please make a note of what it should be below.

**General Surgeon:** Visit duration: 6 hours Visit quantity: 24 per contract year Orthopedic Surgeon: Visit duration: 6 hours Visit quantity: 12 per contract year Urologist: Visit duration: 6 hours Visit quantity: 4 per contract year Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.
