List CLINS, item descriptions, quantities and units of measure: 0001 - Biological Indicator Ampoules; Qty: 3,000 EA 0002 - Shipping/Freight/Delivery; Qty: 1 EA vi. Description/Statement of Work/Specification: See Attachment 2. vii. Date(s) and place(s) of delivery and acceptance: Delivery within 365 days of award to 1525 Discovery Way, Manhattan, KS 66502. This requirement shall be FOB Destination. viii. FAR Provision 52.212-1, Instructions to Offerors-Commercial, is incorporated by reference and applies to this acquisition.
